Dog Adoption Application

The information you provide on this application will assist CCHS in making a possible match between you and a dog available for adoption in our foster program. Please be detailed in your answers. Failure to provide all of the necessary information may result in delay or rejection of your application.

Applications will be reviewed and a response will be e-mailed within 7 days of receiving your request on a specific animal. If the application is a general request for a dog, it will be retained for 6 months. You will be notified if a match becomes available in that time. Please inform us if you want your application withdrawn at any point.

CCHS is a non-profit group and depends on donations to continue operating. The adoption fee is used to cover some of the costs of fostering and vetting the animals up to the time of their adoption.

CCHS reserves the right to refuse any applicant for any reason. The above information is used solely for the purpose of finding an appropriate home for the CCHS foster dogs. The information will not be used for any other purpose

CCHS is in no way liable or responsible for any damage, accident, or injury resulting from the placement of a dog into your household.

This application will remain on file for 6 months.
